Second chance to take the 2011 Community Health Assessment!

Norwood Health Commissioner Brad King asked us to repost the link to the 2011 Community Health Assessment to increase feedback from community members. If you did not respond to the survey when we posted it this past July, please take a few minutes to fill it out this time around. Survey responses are used to help NHD/the City qualify for many grants and as well as bring resources to Norwood.
